This volume examines, from both ethical and clinical perspectives, the impact of increasing genetic knowledge on individuals and families. The authors use their experience of working within clinical genetics to describe the social implications arising from the ability to test for an increasing number of genetic diseases. Current controversial issues are described. Several chapters are based on lead articles and editorials the authors have been asked to write for "The Lancet" and the "British Medical Journal". The book provides an introduction to the effects on individuals of the practical issues associated with genetic testing and should, therefore, be of interest to all those involved in clinical genetics.

Dat de mogelijkheden voor prenataal onderzoek zich steeds verder uitbreiden blijft niet zonder gevolgen: sinds iedere zwangere in Nederland een twintigwekenecho krijgt aangeboden, is het aantal zwangerschapsafbrekingen na constatering van bijvoorbeeld een open ruggetje bijna verdubbeld. Vijf jaar geleden werden Maarten Slagboom en zijn vrouw voor de keuze gesteld of zij de zwangerschap wilden voortzetten toen bleek dat hun kind meervoudig gehandicapt geboren zou worden. De zwangerschap werd afgebroken, maar de vragen bleven. Naast de vraag ‘heb ik er goed aan gedaan?’ rezen de vragen wie en wat nu eigenlijk invloed uitoefenen op het hebben van die keuze, en wat de gevolgen ervan zijn. Maarten Slagboom interviewt in dit boek artsen, politici, wetenschappers en andere betrokkenen, en hij beschrijft zijn persoonlijke ervaringen. Hij legt bloot hoe het samenspel van voortschrijdende technologie, politieke compromissen en overtuigingen van medisch specialisten en ethici steeds meer ouders voor genadeloos ingewikkelde dilemma’s plaatst. Le diagnostic prénatal a trente ans, l'âge de la maturité. Au cours de ces années, de remarquables progrès ont été accomplis. Au début des années 1970, la découverte d'une malformation était le plus souvent une mauvaise surprise lors de l'accouchement. Les anomalies chromosomiques, les maladies géniques, les fœtopathies n'étaient découvertes que plus ou moins tardivement après la naissance. De nos jours, la situation a fondamentalement changé. Le fœtus est devenu un patient à part entière. Les techniques de prélèvement de cellules fœtales se sont diversifiées et, conjointement, se sont développées des techniques de laboratoire de plus en plus élaborées, qu'il s'agisse de biochimie, de cytogénétique ou de biologie moléculaire, dont l'application nécessite une étroite collaboration entre les divers praticiens impliqués. L'échographie est devenue de plus en plus performante. II n'est pas jusqu'à des traitements médicaux et chirurgicaux qui ne soient appliqués au fœtus. Plus récemment encore, les progrès en génétique humaine et en imagerie, le diagnostic préimplantatoire et l'analysé de l'ADN fœtal dans le sang maternel bouleversent les stratégies appliquées. Toutes ces transformations ont eu de considérables conséquences éthiques, économiques, sociales et juridiques, dont les médias se font régulièrement l'écho. Pour faire le point sur ces avancées, Marc Dommergues, Ségolène Aymé, Paul laniaud et Valérie Seror, coordinateurs scientifiques de ce livre, ont fait appel à des auteurs de disciplines diverses (médecins cliniciens et biologistes, chirurgien, psychologue, économiste, juriste, éthicienne) dont les noms font autorité en la matière. Chacun d'eux a écrit, avec le fruit de sa longue expérience, des chapitres précis, très complets, techniquement très fouillés, assortis des textes législatifs et réglementaires en vigueur. Tous les aspects du diagnostic prénatal ont été envisagés. Après une introduction, sont traitées les indications et la mise en œuvre des techniques actuellement utilisées, les stratégies de dépistage, les conséquences obstétricales et pédiatriques, et l'organisation du dépistage et du diagnostic prénatal en France. Le livre se termine par une synthèse très claire. Cet ouvrage sera non seulement une référence pour les spécialistes, mais aussi pour les médecins d'autres disciplines curieux de s'informer et pour les biologistes cherchant à mieux comprendre les enjeux cliniques de leurs actions. Il sera également utile à tous ceux qu'intéresse, à un titre quelconque, le diagnostic prénatal : politiques, magistrats, sociologues, psychologues, philosophes, théologiens, assureurs, qui pourront y trouver, au gré des chapitres, des réponses à leurs interrogations. En fait, ce livre est un véritable condensé de tous les efforts et de toutes les recherches effectuées à ce jour dans le domaine du diagnostic prénatal.

As prenatal tests proliferate, the medical and broader communities perceive that such testing is a logical extension of good prenatal care—it helps parents have healthy babies. But prenatal tests have been criticized by the disability rights community, which contends that advances in science should be directed at improving their lives, not preventing them. Used primarily to decide to abort a fetus that would have been born with mental or physical impairments, prenatal tests arguably reinforce discrimination against and misconceptions about people with disabilities.In these essays, people on both sides of the issue engage in an honest and occasionally painful debate about prenatal testing and selective abortion. The contributors include both people who live with and people who theorize about disabilities, scholars from the social sciences and humanities, medical geneticists, genetic counselors, physicians, and lawyers. La biologie moléculaire rend actuellement possible de prédire plus ou moins exactement, à travers une analyse génétique, si une personne développera plus tard dans sa vie une maladie héréditaire. Deux maladies héréditaires graves se manifestant vers l'âge de 40 ans et actuellement prédictibles sont le cancer du sein héréditaire, et la chorée de Huntington, une affection neurodégénérative progressive, mortelle, caractérisée par des mouvements brusques et imprévisibles (mouvements choréiques). Madame A a son père affecté de la chorée de Huntington. Elle envisage une grossesse. Elle ne compte pas se soumettre au test préalable et en cas de grossesse, n'envisage pas d'accepter un diagnostic prénatal. Madame B est porteuse d'une altération du gène BRCA1 déterminant chez elle un risque de développer un cancer du sein de 80 %. Elle demande un test prénatal afin, le cas échéant, de procéder à une interruption de grossesse. Madame A a-t-elle le devoir de se soumettre elle-même au test (test préalable) et/ou d'y soumettre son enfant à naître (test prénatal), étant entendu que si son enfant est porteur du gène de la chorée, cela implique irrémédiablement qu'elle-même est porteuse du gène ? Serait-il éthique, dans l'hypothèse où elle refuse le ou les test(s), et indépendamment de la législation en vigueur, qu'une assurance exclue de prendre en charge les soins liés à la chorée de Huntington pour son enfant ? Madame B a-t-elle le droit d'avoir accès à un test prénatal ? La puissance publique a-t-elle le droit d'influencer et/ou de contrôler le processus reproductif ? Des tests génétiques et/ou une contraception peuvent-ils être obligatoires dans certains cas ? Avons-nous besoin de nouvelles orientations législatives ?

Not just a chronicle of biology’s progress from gene to genome in one hundred years, The Century of the Gene also calls our attention to the surprising ways these advances challenge the familiar picture of the gene most of us still entertain. Keller shows us that the very successes that have stirred our imagination have also radically undermined the primacy of the gene—word and object—as the core explanatory concept of heredity and development. She argues that we need a new vocabulary that includes concepts such as robustness, fidelity, and evolvability. But more than a new vocabulary, a new awareness is absolutely crucial: that understanding the components of a system (be they individual genes, proteins, or even molecules) may tell us little about the interactions among these components.
